Cholangiocarcinoma (CCA) is a highly malignant tumor that originated from the epithelial cells of the bile duct and is divided into intrahepatic bile duct cancer (iCCA) and extrahepatic bile duct cancer (eCCA).iCCA originated in the intrahepatic bile duct system, while eCCA includes hepatic portal bile duct cancer and remote bile duct cancer.The organoid of bile duct cancer mainly comes from the tumor tissue obtained by the patient's surgical resection or biopsy.This type of organoid retains the histological characteristics of the primary tumor (such as positive expression of CK7, CK19, and KRT19), gene expression profile, and genomic landscape, including common driver mutations such as KRAS, TP53, SMAD4, IDH1/2, and FGFR2 fusion.Compared with hepatocellular carcinoma organoid, bile duct carcinoma organoid is more dependent on R-spondin 1, but usually does not require Wnt3a and Noggin to maintain growth.Some iCCA organoids can be self-sustained without exogenous Wnt signals, reflecting the autocrine activation of the Wnt pathway in tumors.
